The Discharge Process

Your care team will confirm you are ready to go home and prepare your discharge summary, prescription and follow-up appointments. Please ensure you understand your medication, wound care and review dates before leaving.

Before You Leave

• Collect your discharge summary and any referral letters.
• Collect prescribed medicines from the pharmacy.
• Confirm any follow-up clinic dates.
• Clear ward dues at the cashier / admissions office.
